We are looking to recruit an ambitions and dynamic Lead Nurse to join our award winning* Continuing Healthcare team in Croydon. You will be responsible for High Cost Care packages and play an integral role within the senior team.

You will need to proactively and independently manage a caseload of high cost patients, facilitate timely assessments and determine ongoing eligibility against the national framework. The role will involve liaising with providers, clients and families in a professional and assertive manner and be competent and confident to have difficult conversations about eligibility for CHC.

You will work collaboratively with the Placement team when negotiating reductions in costs with providers for those that remain eligible for CHC and produce a high standard of reports that accurately reflects the needs of the patient and service.
