We are currently recruiting  two full time and one part time qualified Registered Nurses, Registered Mental Health Nurses and Registered Learning Disability Nurses to join the Continuing Healthcare Team based in either Ashford but working across Kent. You will be able to further develop your assessment skills and have autonomy in your work.

You will assist in the implementation and delivery of the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care and will be able to apply it within the context of the assessment role. You will be responsible for facilitating assessments and reviews for patients and , arranging appropriate care. You will develop and influence the quality of care that is provided to patients; drawing on previous skills and knowledge to improve patient experience of care.

You will need to demonstrate excellent assessment and communication skills; have relevant experience in complex case management and a basic understanding of the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care although we will offer full training.
