Do you have experience of case management and/or conducting assessments and the ability to chair and co-ordinate meetings?

If yes, then NEL (acting on behalf of NHS West Kent CCG, NHS Ashford CCG, NHS Canterbury & Coastal CCG, NHS South Kent Coast CCG & NHS Thanet CCG) would love to hear from you!

The NEL Placements team is looking for 3 full time (37.5 hours per week) CHC Assessors to join the team.

Why would you want to be a CHC Assessor?

As a CHC Assessor you will support the screening and assessment of individuals applying for and already in receipt of CHC/FNC packages of care. You will be responsible for co-ordinating the most vulnerable people in society to ensure they are in receipt of the correct care. You will have the opportunity to liaise and work closely with Social Services, providers, patients and their families and other relevant agencies in assessing and supporting individuals, as well as developing care plans and review processes and making changes to care packages in accordance with the needs of the individual.

You will work within a strong, friendly and supportive team, playing a key role in making recommendations on eligibility for CHC and FNC packages as well as supporting the Operational Managers in assisting and supporting the management and implementation of the national frameworks and other national policies as they relate to CHC/FNC. Key responsibilities of this role could include conducting assessments, case co-ordination, decision making, and dealing with appeals.

To be successful in this position, it is essential that you are:

  • Experienced in case management and patient assessments.
  • Experience in chairing and co-ordinating meetings in a variety of settings.
  • Managing highly emotional situations which may result in challenge/conflict.
  • Experienced in working within and supporting a multi-disciplinary team.
  • An excellent communicator with evidence of advanced verbal and written communication skills.
  • Experienced in using tact and diplomacy, with demonstrable problem solving skills.
  • Dedicated to providing high standards of patient care.

It is highly desirable that candidates should have knowledge of the Mental Capacity Act and Mental Health Act and an understanding of the current CHC framework, guidance and policies, however this is NOT essential as a full induction with on-going training and support will be provided, including the opportunity to access the new CHC e-Learning programme.
