This role is for a Band 5 Funded Nursing Care (FNC) Nurse Assessor. You will be an established clinician or a newly qualified nurse with community experience. You will be able to work with patients and families, to support the assessment and review process for FNC.

You will work with families and our local authority partners to undertake comprehensive assessments and reviews, so that eligibility for FNC can be determined. The role will also involve supporting the wider Continuing Health Care (CHC) work (such as 'fast track') within the team and you will work closely with the CHC Nurse Advisors and the administrative staff who support these processes.

You will communicate and develop strong links with clinical services staff, as well as with colleagues from external organisation such as social services, mental health trusts, other CCGs, acute trusts, independent contractors and private care homes / providers. You will also have contact with patients, careers and families.

Liaison is an integral part of the role and will take the form of regular contact by telephone, face-to-face meetings, written and electronic communication and training sessions.

The nature of the role will mean that staff will require a comprehensive induction in FNC and CHC including 'fast track' and successful applicants will be able to access regular supervision. This role will include providing and receiving complex, sensitive information, communicating financial decisions to members of the public and using sound interpersonal skills to overcome barriers to acceptance.

The successful candidate will be a key member of the team supporting and promoting excellent customer care throughout the FNC process, ensuring local resolution of any concerns where possible.
