Enfield Clinical Commissioning Group are looking for an exceptional Mental Health Nurse to oversee their Complex Care Pathway, and manage referrals and Discharges for Female PICU in Adult Mental Health Services. This will also include planning aftercare arrangements for Section 117 and Section 37/41 Patients stepping down from a hospital setting.
You will be expected to have a sound experience in understanding Acute Adult Mental Health, both within Inpatient, and in a community setting.
A good understanding is also required of managing complex cases and working in partnership with other Multi-professional Agencies in the NHS, Private and Independent Sectors to ensure that the current and future needs of the Individual Patients needs are met.
The candidate will need to be good at prioritising and managing their time. . You will be expected to urgently review Patients at very short notice, and make assessments around their need for a Female PICU, or Complex Care Rehabilitation Service.
The post holder will also act as a clinical nurse advisor around Mental Healthcare for Enfield Clinical Commissioning Group.
They will be expected to be involved in various forums and attend meetings on behalf of the Trust, where a view around Mental Health care is being sought.
The post holder will be attached to the Commissioning Directorate, but will be provided with Clinical governance support from the Continuing Healthcare Team, also based within Enfield Clinical Commissioning Group.
