Continuing Healthcare Nurse Assessor:

Are you looking for a change and a new challenge? Due to the expansion of the Continuing Healthcare Team an exciting opportunity has arisen for an experienced nurse to join our busy and dynamic Team.

You will need to be highly motivated and enjoy co-ordinating and communicating with a wide range of people. You should be highly organised, confident to work alone or as part of a team, have good interpersonal skills and a flexible approach to work.

Knowledge of the Continuing Healthcare / Funded Nursing Care Framework is essential as you will work under the guidance and support of the Head of Continuing Healthcare to implement the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care for residents in West Essex.

This role identifies and assesses the primary healthcare needs of patients to determine whether they are eligible for funding either through Continuing Healthcare or Funded Nursing Care.

You will promote best practice and provide professional advice to nursing homes and other providers, promoting good relationships and liaising with colleagues from other disciplines both internally and externally to the CCG.
